Paramount has disease management programs to help you. Paramount’s disease management team is committed to improving the health status and quality of life of our members.
Each of our programs includes health coaching services, case management evaluation and services, pharmacy services, educational mailings, web-based interactive tools and on-line support. Each program has been designed to support the medical care provided in the office and is based on established guidelines.
