Once a provider is selected, a map can be generated to show the location.

Each of Paramount's Ohio and Michigan products (Ohio HMO, Elite, PPO, etc.) has its own provider list. Therefore, you must choose a product line prior to searching for a provider.  All other selections are optional; however, additional information is helpful in narrowing the search.
